There are different forms and certificates in use in the visa process. Here can you find a summary of them:

1. Visa application form

1.1. Short stay visa (Schengen Member States)

The visa application form is free and must be completed in one of the following languages: Dutch, English, French. (Please refer to the important point 2, regarding the use of language).

The visa application must be signed and dated (two times, at the point nr 37 AND on the last page!) by the applicant.

1.2. Long stay visa (Belgium)

The visa application form is free and must be completed in one of the following languages: Dutch, French, English. (Please refer to the important point 2, regarding the use of language).

1.3. A recent passport size photograph should be affixed. The technical requirements for the photograph shall be in accordance with the international standards as set out in the International Civil Aviation Organization (ICAO) document 9303 part 1, 6th edition. click here 

2. Use of language

The applicant has the opportunity to choose one of our three national languages used in Belgium in which the application will be treated.

2.1. Short stay visa (Schengen Member States)

Your preference will be determined by the application form you choose to complete: French, Dutch or German. If you choose to complete the English application, it will be considered that you have no preference.

2.2. Long stay visa (Belgium)

You must clearly indicate your preference by completing one of the following forms: in French or in Dutch or in German. In order to help you, you can find here a translation in English.

3. Certificate confirming that the joined person has a medical insurance valid for Belgium for himself and all his family members

In case of family reunion or cohabitation with a non-citizen of a European Member state nor of the EEA, one of the following certificates must be completed by the social security office of the joined person:

4. Medical certificates

For long stay visas/Marriage/Cohabitation :

A medical certificate attesting that you are not carrying any of the diseases which might endanger public health; This certificate must be no older than 6 months from the date of issuance. The specific form must be issued, signed and stamped by one of the Embassy’s agreed physicians. All the analysis, reports and X-rays undergone by the applicant must be provided along with the medical certificate at the time of the application.

For short stay medical visa :

A medical certificate indicating the exact medical condition of the patient and stating that the medical treatment is not available in the country. The specific form must be issued, signed and stamped by one of the Embassy’s agreed physicians All the analysis, reports and X-rays undergone by the applicant must be provided along with the medical certificate at the time of the application.

(*) Please note that the medical certificate submitted with your file can only be in French or in Dutch. The translations in English is provided only in purpose to help you and your doctor

In your work permit process, the Belgian authorities might ask you to complete another medical certificate. The form is available here. We recommend you to ask your doctor to complete bothmedical certificates: for visa matter (b) AND for work permit.

5. Letter of guarantee in the prescribed form

There are 3 types of prescribed forms of letter of guarantee. They are to be collected at the city hall where the signature of the sponsor has to be legalised. Here are the samples:
